COMMUNITY-BASED REHABILITATION (CBRS)
Our Community-Based Rehabilitation (CBRS) program helps individuals build the practical skills and confidence needed to live more independently and engage more fully in their communities. Designed for those managing mental health challenges, CBRS focuses on real-world support that complements clinical therapy.
CBRS services are goal-oriented, skill-building, and client-centered. Working one-on-one in the community or home environment, our specialists help clients develop and strengthen the abilities needed for daily living, wellness, and meaningful connection.
Our CBRS providers work in close collaboration with each client’s therapy and care team to ensure a cohesive, supportive approach. The goal is to empower individuals to not just manage symptoms—but to thrive in everyday life.

CASE MANAGEMENT
Our Case Management services are designed to help clients navigate the complex challenges that often accompany mental health and life transitions. We understand that healing and stability require more than therapy alone—they require access to the right resources, systems, and ongoing support. Our case managers serve as advocates, connectors, and guides—working alongside each client to create a pathway toward greater stability, independence, and well-being.
Case Management at Holos is individualized and collaborative, tailored to meet the unique needs and goals of each person we serve. Whether you're managing a mental health diagnosis, recovering from addiction, or working through housing, employment, or healthcare needs, our team is here to help reduce barriers and build sustainable support systems.
Our case managers work closely with your therapist and any other providers to ensure care is integrated, holistic, and aligned with your healing journey. We believe everyone deserves the support they need to live with dignity, purpose, and hope—and we’re here to help make that possible.
Our Case Management services may include:

RESPITE CARE
Caring for a loved one with mental health or behavioral challenges is a meaningful role—but it can also be physically and emotionally exhausting. At Holos, we’re here for the whole system—supporting not just individuals, but the families and caregivers who walk beside them.
Respite care is available under Idaho's Youth Empowerment Services (YES) program. Respite is a short-term, in-person service designed to provide temporary relief to parents or legal guardians of children under 18 with a Serious Emotional Disturbance (SED). This service aims to reduce caregiver stress and prevent burnout by offering a break while ensuring the child receives appropriate care in a safe environment. This program aligns with the YES Principles of Care, emphasizing family-centered, individualized, and community-based support.
Respite care is more than just a break—it’s an opportunity for everyone involved to recharge, reset, and receive the support they need. We recognize that caregiving is a long-term journey, we can help sustain that journey—providing balance, reducing burnout, and helping families maintain healthy, supportive relationships.
Key Aspects of YES Respite Care:

Eligibility: Available to families of children under 18 diagnosed with SED, as determined through an assessment process.

Service Providers: Care is delivered by trained respite caregivers familiar with the child's needs, ensuring continuity and safety.

Usage: Intended for short-term relief, allowing parents to attend appointments, manage personal matters, or rest, rather than serving as a substitute for regular childcare.

PEER SUPPORT
At Holos, our Peer Support services offer connection, understanding, and encouragement from individuals who have walked similar paths. Peer Support Specialists use their own lived experience with mental health, recovery, or life challenges to offer hope, build trust, and empower others on their journey toward healing and growth.
Unlike clinical services, peer support is rooted in mutuality and shared experience. It’s about being seen, heard, and supported by someone who truly understands—someone who has been there and is now thriving. Peer support complements traditional therapy by adding an additional layer of empathy, real-world insight, and practical guidance.

Peer Support Specialists are certified and trained to maintain healthy boundaries, active listening, and trauma-informed care. They work in collaboration with your care team to support your goals in a non-clinical, empowering way.
